Thank you for your interest in a Shiloh Shepherd puppy from Shilohs of Kai-Lyn. In order to meet our initial requirements, you must:

• be 21 years of age or older and eligible to enter into a legal contract;

• complete this application form in full, supplying all pertinent contact information;

• have the agreement and full participation of your family;

• be able and willing to spend the time and money necessary to provide training, veterinary care, daily exercise and a nurturing home for your puppy.

This form is a Puppy Application -- not an agreement to purchase a puppy. Your completed application will provide the information we need to eventually select the best puppy for you, one who will fit in well with your family, lifestyle and expectations. The completion of this application is the first step for us to get to know you, because as your breeder we will be working together very closely over the coming months and years.

As a means of producing the best Shiloh Shepherds we can for generations to come, health testing is extremely important to the future of our breeding program. Shilohs of Kai-Lyn expects every puppy buyer to agree to performing a set criteria of health tests once your dog reaches two years of age. This stipulation stands regardless if a dog is sold as a pet or as show/breeding quality, and is a standard practice for all reputable breeders.
